Required Documentation:

- Original Birth Certificate

- Social Security Card

- Current Immunization Record Signed by a Doctor

- Completed Certificate of Vision, Hearing, Dental and Nutrition

- Two Proofs of Residency:  Water/Power Bill - Utility Proof & Lease, Bank Stmnt., Renters Insurance, Paystub, Mortage, Copy of Section 8, State Drivers License or  Current HOA Residency Proof.

- Government Issued Picture ID

Once online registration is complete, please email Stephanie Sydnor at sydnors@fultonschools.org with your name, your child's name, date of birth, Grade level and application number.
