Student Registration Information for the 2022-23 school year
Parents must complete and submit the Fulton County Enrollment Form online
Please visit this page to read about registration requirements, including residency and documents needed.
The Online Registration form allows documents to be directly uploaded while completing the application.
When the OLR application has been completed, please provide the application number to the Northwestern Middle School Registrar, Maria Gavenchak at firstname.lastname@example.org
Once the application is reviewed and all required documents are provided, the school will approve the application. Your student is NOT enrolled until this step is completed.
Please note that OLR applications submitted prior to August 1 will NOT be approved without ALL necessary documents. This includes 2 proofs of residence and the 2 Georgia health documents (Georgia Immunization form 3231 and Georgia Certificate of Vision Hearing Dental Nutrition Screening form 3300).
