Learning Environment Hardship Form
Changes to the selected learning environment for the 3rd nine weeks of the school year cannot be changed except through the hardship application process. If you wish to change your learning environment, please download and complete the Learning Environment Hardship form (above) and scan/email it to Mr. Adel@ adel@fultonschools.org or drop off a hard copy of the form to the front office at the school. Once a decision is made at the Zone Superintendent's office, you will be contacted with the decision. Until a decision is made, you must remain in your current learning environment.

Hooch PLUS - Student Tutoring Program
PLUS ( Peer-Led Upperclassmen Studying) is a program designed to supplement in-class learning with weekly group/one-on-one study sessions. PLUS leaders will reinforce lecture material, answer questions and provide homework help during these sessions.

Step 3. Gather and bring the following:
From Previous School:
- Copy of Transcript (rising 9th graders must provide final transcript from middle school)
- Discipline Record (verification of no outstanding disciplinary action)
- Attendance Report for previous school year
- Withdrawal Form from previous school with withdrawal grades if applicable
- Complete mailing address of the previous school attended
- Mathematics Documentation (not required for enrollment but needed for proper placement)
From Home: Residence Verification: two proofs of residence from the school system's approved list of verifiable residence documents- (1) one from the approved Utility List: current water or electricity bill
- (2) one from the approved Residency List: Apartment/House lease, Drivers License w/current address, copy of Home Contract, Homeowners/Renters Insurance Card, current paycheck stub, current bank statement, current HOA bill/statement, copy of Home Mortgage payment, copy of Section 8/HUD Housing Contract
- Georgia Certificate of Immunization Form 3231 and Georgia Eye, Ear, and Dental Screening Form 3300 (these forms may be obtained from a Georgia physician, walk-in clinic, or the North Fulton Regional Health Center located at 3155 Royal Drive, Alpharetta, GA 30022)
- Student's Social Security Card
- Copy of Student's Birth Certificate
- Parent/Guardian Driver's License/State-Issued ID/Passport
- Legal Guardianship papers (if not living with natural parent)

Withdrawing a student from Chattahoochee High School
Due to the current Covid-19 guidelines: parents or Custodial guardians should not visit the school to withdraw their students
