Transition Services

Transition Services support all students with disabilities as they prepare for life after high school. These services help students explore their strengths, interests, and goals while building the real‑world skills they will need for adulthood. This includes opportunities for career exploration, job readiness, independent living skills, and planning for college, training programs, employment, and community involvement. Transition planning is a collaborative process—students, families, and school staff work together to create an individualized plan that supports each student’s unique path. Our goal is to ensure every student has the skills, experiences, and confidence needed to move successfully into adult life.
