Meningococcal Conjugate Vaccine (MCV4) Booster - Due October 6

The due date is October 6
For the 2023-2024 school year, all 11th-grade students must have an updated GA 3231 immunization certificate on file in order to remain active at Cambridge High School. Effective July 1, 2021, children 16 years of age and older, who are entering the 11th grade (including new entrants), must have received one booster dose of the meningococcal conjugate vaccine (MCV4) unless their initial dose was administered on or after their 16th birthday.
Note change: The Georgia Department of Public Health has developed a new Affidavit of Religious Objection to Immunization Form 2208 for use by all new students enrolling in a Fulton County School beginning with the 2015-2016 school year who wish to submit a religious exemption. You can find the new form at the Georgia Department of Public Health under forms and brochures.
GA 3231 immunization forms can be emailed to CAMHSimmunization@fultonschools.org, or left at Bridge West.
