Who Should Register:
Children 5 years old by September 1, 2020, will register for the upcoming 2020/2021 school year. Students coming from another school or system please bring a report card.
Please complete the online enrollment form using the link provided, prior to coming to the school to present your registration documents. You may complete the online enrollment form at the school if necessary.
The following documents are needed to register a student:
- Official State Birth Certificate
- Georgia State Immunization Form 3231 (must not be expired)
- Georgia State Eye/Ear/Dental/nutrition form 3300
- Child’s Social Security Card (waiver available at the school)
- Picture ID (Guardian)
Two Current Proofs of Residency are Required:
One Utility Proof (must be current):
Water Bill
Electric Bill
PLUS
One Residence Proof (must be current):
Copy of home mortgage payment
Lease/Renters Agreement
Current bank statement
Copy of Section 8/HUD Housing Contract
Copy of home contract
Homeowner's/Renter's insurance card
Current paycheck stub
State driver's license/ID with current address
Current HOA Bill/Statement
