TRIP PERMISSION AND RELEASES

I give permission for my child,____________________________________________, to accompany the Centennial High School Orchestra on the Spring Trip/Performance departing on March 29, 2007 and returning on April 1, 2007. I hereby, for myself and for my child, release the Fulton County Board of Education, all school personnel, and all adult chaperones involved in this trip, jointly and separately, from any liability for injury to my child while on the trip, and waive any claim to damages. I have read all attached information and fully understand all rules and regulations regarding the trip.

I fully understand that my child’s personal property, including musical instrument, may not be fully covered by the insurance provided with this trip. I will personally provide any additional insurance coverage I might desire.
