Directions: Please answer “yes” or “no” to the following questions. You will not be awarded a varsity letter without submitting an application. You must fulfill the obligations of your commitment to the various orchestra activities in order to apply for a varsity letter.

Ensemble Activities : (You must participate in all of these activities)

1. Did you participate in Orchestra the entire school year?

3. Did you participate in the GMEA Orchestra Festival?

4. Did you participate in all school and/or community performances?

5. Do you have any unexcused absences from early morning rehearsals or sectionals?

If yes, explain why on back of application.

6. Have you received an orchestra grade lower than ‘B' for any 9-week period?

Individual activities : (You must participate in ‘at least' two (2) of these activities)

7. Do you take private lessons? If so, please provide a signed note from your teacher indicating your participation.

8. Have you participated in Solo and Ensemble Festival, Fulton County Honor Orchestra, GSU Honors Orchestra, UGA Orchestra Festival, Georgia All-State Orchestra, or another comparable individual orchestra activity? 9. Attend two (2) concerts of professional orchestra/string instrument performances during the school year. You must provide the original programs to get credit.
