REGISTRATION TIMES:

	Early Registration for the 2009-10 school year:  Wednesday, May 6, 2009, 8am-4pm, or Thursday, May 7, 2009, 8am-7pm* (Kindergarten / New Student Orientation - Monday, May 11, 2009)
	Registration during the school year:  9:00am-1:00pm (if you register after 11am, the student may not begin class until the following day)
	Summer Registration:   Monday-Thursday, 10:00am-1:00pm

*Students must meet ALL registration requirements to be registered during Early Registration.

NEW STUDENT Registration Requirements:

To enroll at River Eves Elementary School, a student will need:

	Current immunizations** on a Georgia form (must have an expiration date that does not expire before school begins) Immunizations may be transferred to a Georgia form at the County Health Dept (phone: 404-332-1958) or at a Georgia pediatrician's office. ** Note - new laws in effect require a second varicella shot for new kindergartners
	Georgia (Eye, Ear, Dental) form #3300 (may be completed at the County Health Dept or by a GA physician)
	Certified copy of a government-issued birth certificate (hospital registration is not acceptable)
	Affadavit of residency in our school attendance zone (requires picture ID) (affidavit supplied at school or from the FCS website)
	Two proofs of residency in our school attendance zone. Proof of residency may be a signed lease or contract on a house or apartment, a copy of a utility bill (water, gas, or electric), or bank statement.
	Copy of the child's social security card (optional)
	Transfer students only - a copy of the last report card or a document to prove attendance in previous school
