7:15 am-11:15 am

The following information are required to register:

• Grades K-5 Official State Birth Certificate
  
  
• Georgia State Immunization Form (3231)
  
• Georgia State Hearing, Vision & Dental Screening Form (3300)
  
  
• Child's Social Security Number (Optional)
  
  
• 2 types of proof of residence (utility bill, apt. lease, mortgage agreement)
  
  
• Parent Picture I.D.
  
  
• Last Report Card or any  school records from previous school.
  
  
• You will be required to fill out a Affidavit of Residence and have it notarized.  There are separate Affidavits for groups cited in the Board Policy.
