Registration Form
In order to register your child, you need to complete a registration form and
bring it to the office with the accompanying required documents (see Registration
Information).

Release Form (English version) This form must be signed by each
student's parent or guardian in order for students to be photographed or

Bus Permission Form The late bus is to accommodate students
who participate in Intramurals and/or who are kept after school for a school
supervised and sanctioned activity.
