According to Board Policy and Procedure JBC – “School Admissions,” a student residing full-time within the Fulton County School System is eligible to attend a Fulton County school under the following criteria:
- The dwelling in which the student resides must appear on the tax rolls of Fulton County.
- The student resides with his/her parent or legal guardian.
- The student resides with a foster parent through placement by the
- Department of Family and Children Services or a state licensed child placement agency.
- The student is an emancipated minor.
- The student is homeless
To enroll in Fulton County Schools, the student's parent or legal guardian must be a resident of the school system. A notarized Affadavit of Residency is required for each student attending a Fulton County School. You may click here to download a copy.
Ear, Eye and Dental Screening
No child entering a Georgia school for the first time shall be admitted unless the child has a certificate of eye, ear and dental screening. This Georgia Department of Human Resources certificate (Form 3300) must be signed by the Health Department or a licensed Georgia physician.
Immunization Certificate
Georgia requires students to be immunized against measles, mumps, polio, rubella, whooping cough, tetanus, diphtheria, hepatitis B, haemophilus influenza, and varicella. Immunization information must be on the proper Georgia Department of Human Resources form; these are available through the Fulton County Department of Health and Wellness or local physicians (Form 3231).

Birth Certificate
Must be government issued--hospital certificates are not acceptable. Birth certificates for children born in Georgia may be requested from:

Proof of Residency
Per Board Policy JBC, "School Admissions, " two documents indicating residence in the attendance area must be provided for all students*.
Appropriate records for verification of residency must include any two of the following:
- Copy of home mortgage payment book
- Current utility bill (gas, electric, or water)
- Apartment lease showing name of legal guardian
- Homeowner's insurance registration/card
- Current Paycheck stub
- Current Bank statement
- Driver's license showing legal guardian's current address with one of the above
- Copy of sales contract
- Receipt to have utilities connected
- Affidavit of Residence with signatures from the permanent resident and parent or guardian notarized with two of the above. The two additional documents should bear the name of the person signing as the permanent resident of the dwelling or apartment. This form is required prior to enrolling a student and also is required every school year for all students.
*Includes students who are 18 or older; emancipated; adult students. Enrollment of any child who meets the definition of homeless is determined by guidelines in Fulton County Board of Education Policy JBCA.
Social Security Number
Social security numbers are voluntary. Waivers are available in local schools. Applications for social security numbers are also available in local schools.
Migrant number is acceptable.
Academic Records
If possible, please bring most recent report card, withdrawal form from previous school and/or transcripts. These are essential for proper placement of students. Students in grades 7-12 also will need copies of any discipline records.
Special Education
If a child has a current IEP (Individual Education Plan), please bring that in when you are registering your student so that appropriate services can be provided.
In August, parents are again required to submit a notarized Affidavit of Residence form for each child. However, the school board has made the process more streamlined from its previous requirement of all students to submit a notarized affidavit and proof of residence. The purpose of the Affidavit of Residence is to make sure that only students who reside in Fulton County attend Fulton schools. This means that Fulton taxpayers’ money stays in Fulton County to pay for the education of those who live there – instead of students from other counties who attend under false pretenses.
Returning students, except those entering sixth grade or ninth grade, do not need to provide proof of residence but will still complete and submit an Affidavit of Residence form. Proof of residence is required only of students who are (1) new enrollees in a Fulton County school or (2) entering sixth grade or ninth grade. The proof of residence consists of two utility bills in the parent name with the correct address and photo ID of the parent. Our school has a notary available to notarize the form at no charge
Parents have until September 11, 2008 to present this document to school.
