We need your help again this year to make your child's visits to the clinic safe and organized. The clinic will be staffed to meet your child's medical needs Monday through Friday from 8 AM to 4 PM. Prescription medications require you to complete a Fulton County Authorization to Give Medication at School form. A doctor's signature is required on this form for all prescription medications, such as inhalers, antibiotics, and epipens. All prescription medications must be in their original containers with the pharmacy label intact. It is your responsibility to continue supplying the clinic with your child's prescription medications.

Nonprescription medications also require you to complete a Fulton County Authorization to Give Medication at School form, but a doctor's signature is not required. The form must be signed by a parent or legal guardian. Examples of nonprescription medications include vitamins, herbal medications, topical creams, cough drops, cough syrup, Tums, benedryl, Pepto-Bismol, Tylenol, aspirin, Advil, Midol, and flu or cold medicines. All nonprescription medications must be in their original containers with the child's name written on the container.

Hand written notes, faxes, or medication not in its original container will not be accepted for prescription or nonprescription medications in accordance with Fulton County Clinic Guidelines.

If your child has any medical condition of which you would like us to be aware that may require an emergency medical plan, please contact us. Examples are epilepsy, diabetes, allergies to foods, medications, insect bites.

Parents, guardians, or individuals on emergency contact lists will be notified by phone if the child sustains any injury on school grounds. Please provide the school with all home, business, and emergency phone numbers, as well as cell-phone and pager numbers. This will ensure that we are able to contact you in a timely and efficient manner.

Emergency Information

Emergency contact information should be updated by sending the information to school or calling the school office. When you receive a Clinic Emergency Information Card, please update it and return it to the school within five (5) days. Current, accurate information will enable us to contact you. If any information changes during the school year, contact the school immediately. This will enable us to contact you if your child becomes ill or is injured.

Prescription/Nonprescription Medication

Medication time schedules should be set so that when possible, medicine is taken at home rather than at school. However, if medication must be taken at school, the following procedures apply:

1. Prescription Medication - The parent/legal guardian and physician must complete and sign the form entitled "Authorization to Give Medication at School". You can make copies for yourself or request additional forms from the school or print out a copy by going to the link at the top of this page. The completed form must accompany the medication, so be sure to take the form to your physician whenever your child is ill.

2. Nonprescription Medication - The parent/legal guardian must complete and sign the Authorization to Give Medication at School form. The completed form must accompany the medication.

3. The medicine, in the original container (along with the authorization form), must be taken to the school office/clinic for central storage. The parent/guardian should take the medication to school; however, if this is not possible your child should be instructed to take the medication and the authorization form directly to the school office/clinic. Under no circumstances should medication be shown to or shared with other students.

4. At the designated time, the student will go the the clinic to take the medication. Assistance/supervision will be in accordance with the instructions on the authorization form. Medication is a parental responsibility; school employees will not assume any liability for supervision or assisting in the administration of medication.

Student Illness

Sick students who are contagious must not be sent to school. When a student becomes ill at school, the parent/guardian must arrange for the student to be taken home.
