Online Forms

Purpose

Welcome Creek View family members to our experimental forms page. Here you will find links to forms templates created by staff members. Some of the forms will be for schoolwide use to mass collect information from parents and staff. Other forms are for individual classroom use only. Check them out and see if you can use them in your own classroom.

Chances are you may want to MAKE and incorporate forms into your website just as several of your fellow staff member have. You may want to tailor them to your own needs. Here is what you must do:

FIRST: Sign up for a gmail account. I have a gmail account called, CreekViewNews. I use it to send large numbers of images without clogging my mailbox. I use it to turn any document into a pdf file for free. I use it to view create forms and manage the spreadsheets. These are only a fraction of the reasons for having a gmail account. I also have a personal gmail. Once you discover the countless advantages to gmail you will love it! ("I" am Kathy Shields. Helaine Becker is a good one to ask about forms too!)

SECOND: email your new gmail address to Kathy Shields and I will add you as VIEWER. You will be able to open the document in your own account and then save it again making it your very own editable form.

THIRD: Ask for a little help or Google the steps to make or change the form.
