General Information
Students will be sent to the clinic for illness, accidents, injuries and lost teeth. A parent will be notified in the event of vomitting, fever or diarrhea during school hours.
Allergies and Medical Conditions
Please keep the school informed about any allergies or medical conditions. The clinic is responsible for maintaining all student health records. Documentation is entered into a private database as a clinic resource.
Medicine
The medical release form is required for all medications including the over-the-counter medications. It must be signed by parent or guardian in both cases. If it is a perscription medication it also requires a doctors signature.
Changes in Emergency Contacts
Please notify the clinic and the front office staff of any changes in emergency contact information. For your child's safety please notify the school in writing if both parents are out of town and someone else will be acting as guardian in your absence.
