New students and returning students
must reside in Camp Creek Middle School zone. The following documents must
be furnished for all students entering a Fulton County school district for
the first time:
An official copy of the birth certificate.
A Georgia certificate for ear, eye
and dental screening.
A current Georgia certificate of
immunization against diphtheria, pertussis, tetanus, polio, measles, rubella
and mumps.
Proof of residency and in some cases
proof of guardianship.
