Students may register Mon-Thurs from
8:00 a.m. - 10:00 a.m. Parents may pick
up registration materials after 10:00AM from the front office and return to
complete registration during registration hours. Students can only begin school
if they have all the required documents AND are dressed in school
uniforms. Ms. Simpson will notarize
affidavits at no cost to the parent.

Proof of residence (notarized Affidavit of Residence form and
utility bill, lease, mortgage, etc.)

Proof of grade placement (report card, transcript or withdrawal
form from previous school)
·
Complete mailing address of previous school
·
Discipline report from previous school
·
Current immunization certificate (must be Georgia Certificate of
Immunization form 3231)
·
Eye, Ear, Dental Certificate (State of
·
Social Security card
·
Birth certificate
·
CRCT scores for 6th grade students or if new student from another
·
Standardized test scores for students transferring from an
out-of-state school system
Important Notice
All children entering sixth grade (on
or after July 1, 2007) are required to meet the following new requirements:
1. Have two (2) doses of Measles vaccine,
two (2) doses of Mumps vaccine, and one (1) dose of Rubella vaccine or
laboratory proof of immunity against Measles, Mumps or Rubella.
2. Have two doses of Varicella
(chicken pox) vaccine or documentation or disease or laboratory proof of
immunity. At the time your child entered
school only one (1) dose of this vaccine was required.
3. Contact your health care provider to
receive any needed vaccinations and to obtain the Georgia Certificate of
Immunization (Form 3231).
