Please bring the following items to register your student.
Proof of Residency Currently dated official document such as a sales agreement on your home or a lease, or a current utility bill with the parent or guardian's name and address.
Birth Certificate Must be government issued; hospital certificates are not acceptable.
Social Security Number This is voluntary; waivers and applications for social security numbers are available in local schools.
Immunization Certificate Georgia requires students to be immunized against measles, mumps, polio, rubella, whooping cough, tetanus, diphtheria, hepatitis B and varicella. Form 3231 from the Georgia Department of Human Resources is the form used for documenting the immunization status for children who will be admitted to any school in Georgia for the first time on or after August 1, 2000. These forms are available through the Fulton County Health Department or local physicians.
Certificate of Eye, Ear and Dental Screening This screening is required for students who have never been enrolled in a Georgia public school. The information must be on a Georgia Department of Human Resources Certificate and must be signed by the Health Department or a licensed physician.
Academic Records Many schools will send records to us at a later date, but it is extremely important to bring valuable academic information at the time of registration. Bring the most recent report card, transcripts, and any standardized achievement test scores the child has had in the past two years. More complete information will enable us to efficiently provide accurate placement for your child. It is also necessary that you bring the withdrawal form from the current school with numeric grades and current class placement.
Talented and Gifted Placement On school letterhead, the current school administration must state your child is currently enrolled in a Talented and Gifted program and state the criteria for placement. A complete testing history is needed. Have the school supply a list of the courses in which your child was enrolled. Supply placement information and grades in all core curriculum subjects. Special Education Bring the latest IEP from the current school. Contact Ms. Lana Caray, the school IST, for information in this area.

Children's Internet Protection Act
Internet access is available at your school in support of the instructional program. The Fulton County School System complies with the Children's Internet Protection Act. Filtering is in place that blocks access to material that may be inappropriate for your child. Web sites are permitted or denied upon review of the school Media Committee. Acceptable use of the Internet is established in Board Policy / Procedure IFBGA, Electronic Network Access . The policy and procedure may be viewed in the school media center or on-line at "http://www.fulton.k12.ga.us".
