To apply for hardship transfer:
  • Please complete and submit the following application.
    (You may only select one application type)
  • Print the Application for Transfer and obtain any pertinent signatures and/or supporting documentation.
  • Mail/Fax/Hand Deliver completed Application for Transfer to the Student Assignment Office by March 1, 2010 to:
